Carmarthenshire Demonstrator Programme-Brief Information






The Carmarthenshire Demonstrator was managed by a project lead based in the Carmathenshire locality of Hywel Dda Local Health Board at Mynydd Mawr Hospital in Llanelli. All Demonstrator leads worked closely with partners across thier respective areas which encompassess those in primary and secondary care, local authority, the community and the third sector.
