27/05/2011 09:00:00 - Localities Delivering Change Conference-Optic Centre, St Asaph Business Park
The CCM Demonstrator programme national team were present at the Localities Delivering Change Conference at the Optic Centre, St Asaph-North Wales on May 27th 2011. The Conference had a particular focus on locality working-something integral to the workstreams of the demonstrators and featured partners from the healthcare, social services and local government sectors.
15/10/2010 10:30:00 - CCM Demonstrator Learning Event - Cardiff City Hall October 15th 2010
The CCM Demonstrator programme is due to complete its work in 2011 and will be shortly publishing its 2nd Year Report of findings to date. It has carried out wide ranging testing of the elements of the CCM Model and Framework and contributed to the overall transition set out in the models vision. The CCM Demonstrator work is continually being circulated through a series of Learning Papers and Evaluation Reports and both the Minister and Paul Williams are keen to ensure that this learning is applied and implemented across Wales. With this in mind, a CCM Demonstrator Learning Day  took place on October 15th 2010 at Cardiff City Hall to present the learning that can be adopted and the evidence for improving services to senior stakeholders. A key feature of the day was the broader open discussion to further the learning experience and the launch of the Year 2 CCM Demonstrator report, alongside more recent publications. 13/10/2010 09:00:00 - Body and Mind: Making the connections between Chronic Conditions and Mental Health
To Celebrate World Mental Health Day 2010, a workshop event for professionals to learn more about the links between chronic conditions and mental health problems is being held on Wednesday 13th October, 9am - 2pm St Davids Hall, Cardiff. The event will include presentations from Mental Health Professionals and the Expert Patient Programme as well as interactive Open Space workshops, followed by lunch and networking opportunity. 28/09/2010 09:30:00 - Intermediate Care and CCM Community of Practice
A Meaningful Measures for Intermediate Care and CCM Event was held at Hafod Y Hendre, Builth Wells on the 28th of September 2010. This event was a joint CoP with the Intermediate Care CoP to discuss common themes that run across areas of chronic conditions management. The event featured group working with the NLIAH Change Agent Team and also PDSA Cycles to Test Measures session with NLIAH Senior Service Inmprovement Manager Margaret Rennocks.
