During it's running, the Chronic Conditions Management (CCM) Demonstrator Programme was keen to ensure that patients and carers are actively involved in decisions about their care, that they are engaged with across Wales and that their involvement sits at the heart of service planning and delivery. One mechanism to inform this was to interview patients (and carers) themselves and identifying their own personal outcomes and experiences, captured through their personal stories. In conjunction with the University of Glamorgan, the CCM Demonstrators have captured a number of patient stories in different formats-visual, audio and text-from across the three Demonstrator areas. All stories will be able to view on a brand new dedicated website which will be linked from www.ccmdemonstrators.com in June/July 2011.

The Chronic Conditions Management Demonstrator Programme officially closed at the end of March 2011. There are, however, a number of further Learning Papers and Reports due for completion covering areas such as diabetes, co-production, plus locality and multi-disciplinary team working. Each document will be updated and published throughout June and July 2011 and will be available to view at www.ccmdemonstrators.com

The Cardiff Chronic Conditions Management (CCM) Demonstrator is keen to share its innovate learning within the sphere of CCM with a wide as possible audience. With that in mind a series of roadshows are being held across a number of Cardiff hospital sites from the end of January to the beginning of February 2011. The roadshows will consist of a manned displayed stand which will have a variety of reports and also learning, evaluation and development papers available to take away for information. These will provide a wide range of information including 'top tips' and lessons learnt if you want to learn about the work or try and improve services yourself. 19/01/2011 15:45:31 - Carmarthenshire Demonstrator Innovate in Respiratory Care
The Carmarthenshire Chronic Conditions Management (CCM) Demonstrator site has a particular interest in respiratory conditions underlined by the Welsh Assembly's Service Development Directive for Chronic Respiratory Conditions. The work undertaken by Carmarthenshire has seen great strides in health improvement for service users in the locality. This has been duely noted and recognised by thier peers at the recent Medialis Awards at the Hilton Hotel in Cardiff. The Annual Awards Ceremony (in partnership with Welsh Assembly Government's National Institute for Social Care and Health Research (NISCHR),celebrated the remarkable innovations within both Welsh Industry and NHS organisations. Carmarthenshire were presented with the 'Innovation within the NHS Award' which was presented to the Respiratory Unit, Prince Philip Hospital, Llanelli and the CCM Team, Hywel Dda LHB. The winning entry involved the testing of the the feasibility and safety of home telemonitoring for COPD in a semi-rural environment. The Chronic Conditions Management Demonstrator Programme is currently undertaking a survey throughout December 2010 and January 2011 regarding the work of the Demonstrators and their impact on CCM in Wales.  Even if you know very little about the work of the Demonstrators, your completion of the web based questionnaire will help support the design of future Welsh Assembly/NHS programmes and their communication plans. The questionnaire will only take a few minutes and we would like to thank you in advance for your time and interest. Note the questionnaire is available in English and Welsh. The questionnaire can be completed by following this link: http://www.ors.org.uk/surveys/CCMDemonstrators The results of the survey will also be made available in the new year and published on the Demonstrator website for general viewing.
